Director: Immediate actions taken to notify Veteran
In an attempt to notify Veterans of the new federal rule of Hydrocodone combination, letters were inadvertently printed double-sided.
On the front page one unique Veteran’s information was printed and another unique Veteran’s information was printed on the back.
The letters were mailed out on September 15, 2014 and included full name, address and disclosure of the patient currently prescribed a narcotic was accidentally released causing a privacy breach.
“Our goal is to protect both Veterans protected health information and sensitive information,” said Medical Center Director, Marie Weldon. “Immediate actions are being taken to notify the Veterans affected by this incident.”
Veterans who believe they may have been affected by this incident, but have not been notified by VA, may verify whether their information was involved by writing to: South Texas Veterans Health Care System, ATTN: Privacy Officer, 7400 Merton Minter Blvd., San Antonio, Texas 78229.
